The newly enhanced platform provides for chronic care management, transitional care management, and population health management services as well as other care coordination services. In January 2015, the Centers for Medicare and Medicaid Services (CMS) began reimbursing providers who actively manage care delivery for Medicare patients suffering from two or more chronic conditions. These services may be contracted with a third party, such as US CareNet’s NavCare division.
